Our office accepts most major insurance carriers.  As a courtesy to our patients, our staff will file claims with your insurance carrier for you.  Your insurance will determine whether services are covered.  You are ultimately responsible for any costs not covered by your health plan.  Please call your carrier directly for questions about your insurance coverage.

Please bring your current insurance card(s), primary and secondary (if applicable), with you to your appointment.  We will scan your card(s) into our practice management system to ensure we have the correct information to file claims with your insurance company. To make insurance filing more efficient, please inform us of any changes since your last visit in your name, address, phone number, employer, health plan, group or policy number.

Co-payments are due at the time of service. You will be billed for deductibles, coinsurance and any non-covered services after we have received notification from your health plan.
